We have a need for a professional and experienced Intake Coordinator to join our growing team! We are looking for an individual who is detail oriented and wants to combine their interpersonal and organizational skills. Intake Coordinators ensure seamless transition of patients to home care by gathering patient information and history. This person should be able to establish and maintain a positive working relationship with current and potential referral sources.   • High School Diploma or Equivalent required
  • Minimum (1) year of Home Health experience or in a position similar.
  • Knowledge and the ability to apply understanding of medical terminology and basic coding.
  • Knowledge of Medicare home health including, but not limited to Medicare home health admission requirements preferred.
  • Ability to multitask, and prioritize, ensuring the completion of many tasks, projects, and reports in a fast-paced environment.
  • Intermediate functional knowledge of computer/technology skills including, but not limited to MS Office, MS Excel, navigation of company website, and electronic medical record.
  • Ability to exercise independent judgment, work well with marketing and clinical team, and communicate with referral sources confidently and knowledgably.
